Consent to Participate

I give permission for my child to participate in ZIKSA programs, including Holmes Club sessions, Incubator conversations, off-site visits, workplace experiences, and related educational activities.

I understand that participation may involve ordinary risks associated with travel, walking, third-party facilities, equipment, demonstrations, group activities, and real-world workplace environments.

Assumption of Risk & Release

I voluntarily allow my child to participate and, to the fullest extent permitted by California law, assume the ordinary risks associated with participation.

I release ZIKSA INC., its directors, officers, employees, volunteers, and authorized representatives from claims arising from ordinary negligence related to participation in ZIKSA activities.

This waiver does not apply to gross negligence, reckless conduct, intentional misconduct, or liability that cannot legally be waived.

Emergency Care

If I cannot be reached in an emergency, I authorize ZIKSA representatives to contact emergency medical services and obtain reasonable emergency care for my child.

Student Safety & Conduct

I understand that my child must follow ZIKSA and host-site safety instructions, remain with the assigned group, respect restricted areas, and behave appropriately.

Transportation

Unless otherwise stated in writing, I understand that I am responsible for arranging transportation to and from ZIKSA activities.

Final Acknowledgment

By submitting this form, I confirm that I am the parent or legal guardian of the student named above and that I have read and agree to the terms provided.
